En la búsqueda de empleo, el diagnóstico de salud mental pesa desde la casilla de salida: solo el 27,3% de las personas con discapacidad psicosocial tiene empleo o lo busca.
El 88% de las personas con discapacidad por salud mental asocia el empleo con mejor calidad de vida y bienestar; sin embargo, el 82,3% vive la búsqueda de trabajo con ansiedad o temor a ser rechazado.
· En líneas generales, el empleo, cuando se desarrolla en condiciones ambientales, psicológicas y socioeconómicas favorables, actúa como un factor de protección de la salud mental, ayuda a prevenir posibles trastornos y ejerce un efecto terapéutico y estabilizador. Así lo percibe el 88% de las personas con discapacidad por motivos de salud mental, quienes destacan que trabajar aporta rutina (72,2%), estabilidad económica (70%), sentimiento de utilidad (59,7%), autoestima (51,1%) y fortalecimiento de las relaciones sociales (35,4%).
· Sin embargo, el 82,3% de las personas con discapacidad de tipo mental afronta dificultades emocionales en la búsqueda de empleo, sobre todo ansiedad o bloqueo (75,9%) y miedo al rechazo (61,6%), seguidos de problemas de concentración (52%) y limitaciones técnicas (19,6%).
· Durante el itinerario de búsqueda de empleo, el diagnóstico condiciona desde el primer paso. Casi la mitad de las personas encuestadas (47,1%) percibe expresiones excluyentes en las ofertas de empleo. Por ejemplo, fórmulas como “trabajar bajo presión”, “tolerancia al estrés” o “disponibilidad total”, desincentivan a candidatos/as cualificados/as, que acaban autoexcluyéndose por temor a no cumplir expectativas.
· El 58,4% de las personas encuestadas afirma haber experimentado en algún momento preguntas estigmatizantes o poco pertinentes en entrevistas de trabajo relacionadas con su salud mental. Por ejemplo: ¿Cómo afecta la medicación a tu concentración? ¿Cuántas bajas has tenido como consecuencia de tu discapacidad?
· La falta de una acogida adecuada y de un acompañamiento efectivo en el puesto de trabajo impacta directamente en la seguridad psicológica, hasta el punto de que un 66% de las personas con discapacidad de tipo mental que trabaja teme perder su empleo.
· Los frenos más frecuentes para que las personas con discapacidad psicosocial (como consecuencia de un diagnóstico de salud mental) se consoliden en su puesto de trabajo son la falta de claridad de funciones, acompañamiento o rol (55%), seguido de la gestión inadecuada de la carga de trabajo (27,8%).
· “Cuando el proceso de búsqueda de empleo se desarrolla en un entorno adecuado, con una acogida respetuosa, acompañamiento y un trato inclusivo, las personas con discapacidad por motivos de salud mental tienen la oportunidad de desplegar su potencial y fortalecer su autoestima. El empleo constituye, en este sentido, un recurso de gran valor para mejorar la salud mental y favorecer la inclusión social y laboral. No obstante, la aparición de barreras, tanto en la búsqueda como durante el desempeño (acogida insuficiente, gestión inadecuada de la carga de trabajo o entornos poco empáticos) puede convertir la experiencia en un factor de riesgo. Por ello, resulta imprescindible adecuar los procesos de selección e impulsar culturas laborales más humanas y accesibles que garanticen la igualdad de oportunidades y protejan, en lugar de deteriorar, el bienestar psicológico de las personas candidatas y trabajadoras con discapacidad psicosocial”, declara Begoña Bravo, directora de Inclusión de la Fundación Adecco.
Próximo al Día Mundial de la Salud Mental, el Observatorio de la Vulnerabilidad y el Empleo de la Fundación Adecco, presenta la segunda edición del informe Discapacidad, Salud Mental y Empleo, un
análisis realizado con el apoyo de 31 empresas comprometidas 1 , que recoge la voz de las personas con discapacidad derivada de un diagnóstico de salud mental, con el objetivo de visibilizar las barreras que encuentran a lo largo de todo el itinerario de empleo e impulsar acciones que contribuyan a minimizarlas.
El informe parte de la evidencia de que el empleo, cuando se desarrolla en condiciones ambientales, psicológicas y socioeconómicas adecuadas, constituye un factor de protección de la salud mental, ayuda a prevenir posibles trastornos y ejerce un efecto terapéutico y estabilizador. Esta premisa queda respaldada por la evidencia que aportan las respuestas de las personas encuestadas, como presentaremos más adelante.
Las conclusiones de este análisis se apoyan en una encuesta a 290 personas con certificado de discapacidad psicosocial, esto es, por un diagnóstico de salud mental (esquizofrenia, depresión crónica, trastorno bipolar u otras patologías) así como en entrevistas a un equipo multidisciplinar de especialistas en inclusión laboral y salud mental de la Fundación Adecco, complementadas con fuentes oficiales de referencia.
Estigma y autoestigma: la doble barrera al empleo
En España, se contabilizan 315.200 personas con certificado de discapacidad por diagnóstico de salud mental en edad laboral, lo que representa el 16,2% del total de personas con discapacidad de 16 a 64 años. De ellas, solo un 27,3% (86.200 personas) es activa -es decir, tiene empleo o lo busca-, el porcentaje más bajo de participación laboral entre todos los tipos de discapacidad (la media es del 35,5%). En la misma línea, la tasa de empleo apenas alcanza el 19%, lo que significa que el 81% de las personas con discapacidad como consecuencia de un diagnóstico de salud mental, en edad laboral, no está trabajando. Esta cifra resulta especialmente significativa si la comparamos con la media del resto de las discapacidades, donde la proporción de personas sin empleo se sitúa en el 71,5%. Es decir, la
brecha de participación laboral para las personas con discapacidad psicosocial es aún más acusada, consolidándose como el grupo con menor inclusión laboral dentro de la discapacidad.
Las bajas tasas de actividad y empleo entre las personas con discapacidad derivada de un problema de salud mental se explican, en gran medida, por la persistencia del estigma social, que alimenta prejuicios y estereotipos sobre su capacidad, fiabilidad o incluso seguridad. Estos sesgos no solo condicionan la percepción externa y las oportunidades de contratación, sino que también pueden interiorizarse en forma de autoestigma, generando un círculo de aislamiento, baja autoestima y renuncia anticipada a procesos laborales. De este modo, el estigma social actúa como barrera externa y el autoestima como obstáculo interno, reforzando mutuamente su impacto y limitando de manera significativa las posibilidades de inclusión laboral.
“Con frecuencia, las discapacidades de tipo mental se asocian de forma muy reduccionista con inestabilidad emocional, falta de capacidad para asumir responsabilidades o incluso imprevisibilidad en el desempeño. Estos estereotipos simplifican y distorsionan la realidad, pues ignoran tanto la diversidad de diagnósticos como la posibilidad de contar con tratamientos, apoyos y entornos laborales adaptados que permitan a las personas desarrollar su talento con plenas garantías. El resultado es una barrera invisible pero muy poderosa: la desconfianza social, que se traslada al ámbito empresarial, limitando las oportunidades y reforzando la exclusión laboral”, señala Cristina Leirós, coordinadora del programa de Salud Mental en la Fundación Adecco.
El empleo como factor de protección de la salud mental
Según revelan los resultados de la encuesta, el empleo no solo constituye una vía de inclusión social y económica, sino que, cuando se desarrolla en condiciones ambientales, psicológicas y socioeconómicas adecuadas, se convierte en un elemento decisivo para la protección de la salud mental. Su valor trasciende lo laboral, aportando estabilidad y actuando como motor preventivo, terapéutico y estabilizador. Así lo constatan el 88% de las personas con discapacidad psicosocial encuestadas, que destacan, entre los principales beneficios, la rutina (72,2%), la estabilidad económica (70%), el sentimiento de utilidad (59,7%), la mejora de la autoestima (51,1%) y el fortalecimiento de las relaciones sociales (35,4%).
A pesar de ello, cuando estas condiciones ambientales, psicológicas y socioeconómicas adecuadas no se garantizan a lo largo del itinerario de empleo, el trabajo pierde su capacidad terapéutica y estabilizadora, pudiendo incluso reforzar el estigma y generar nuevas barreras para la inclusión. Lo analizaremos en los siguientes apartados.
La carga emocional añadida en la búsqueda de empleo
A pesar de que el empleo constituye un elemento esencial para normalizar la vida de las personas con discapacidad como consecuencia de un diagnóstico de salud mental, el proceso de búsqueda plantea retos emocionales y personales para ellas. De hecho, el 82,3% de las personas encuestadas reconoce haber atravesado alguna dificultad durante este proceso, lo que refleja la importancia de acompañar y apoyar estos itinerarios.
Entre las situaciones más frecuentes destacan la ansiedad o el bloqueo emocional (75,9%) y el miedo al rechazo o a la discriminación (61,6%), que muchas veces no responden tanto a experiencias previas concretas como a la anticipación de posibles prejuicios. También aparecen la dificultad de concentración o expresión (52%) y, en menor medida, las dificultades técnicas (19,6%), relacionadas con el manejo de plataformas digitales o la familiaridad con los procesos de selección.
Solo un 17,7% afirma no haber tenido ningún obstáculo, lo que indica que la mayoría de las personas con discapacidad psicosocial encara la búsqueda de empleo con mayor carga emocional, que podría reducirse si contaran con más experiencias previas positivas y con procesos de selección más accesibles e inclusivos.
Obstáculos tempranos en la búsqueda de empleo: ofertas excluyentes e insuficiente retroalimentación
Desde la casilla de salida, el diagnóstico pesa en la búsqueda de empleo. Así, un 47,1% de las personas
encuestadas percibe expresiones excluyentes en las ofertas, frente a un 30,7% que las considera accesibles y un 22,1% que mantiene dudas. Ejemplos frecuentes son: ‘capacidad para trabajar bajo presión’, ‘tolerancia al estrés’, ‘buscamos personas resilientes, capaces de adaptarse a elevados ritmos de trabajo’, ‘entornos de alta exigencia emocional’, ‘disponibilidad total” o ‘habilidad para mantener el rendimiento en situaciones de tensión’. A esta barrera se suma la falta de retroalimentación: solo un 23,3% recibe feedback adecuado, mientras que la mayoría se enfrenta a silencios o respuestas genéricas que aumentan la frustración y la sensación de invisibilidad.
“Cuando una oferta presenta la ‘tolerancia al estrés’ o el ‘trabajo bajo alta presión’ como rasgos identitarios de la posición, muchas personas con discapacidad por motivos de salud mental perciben que no son bienvenidas. Conviene recordar que la tolerancia al estrés no es realmente una competencia, sino un riesgo psicosocial, y enunciarla como requisito puede reforzar el autoestigma y llevar a la autoexclusión antes incluso de postular”, destaca Jordi Navarro, consultor de Inclusión especialista en Salud Mental de la Fundación Adecco.
Entrevistas de trabajo con barreras añadidas
La fase de selección es clave para valorar las competencias de una persona candidata, pero en el caso de los profesionales con discapacidad psicosocial, suele verse empañada por preguntas que revelan prejuicios y desconocimiento. Es el caso de aquellas en las que se indaga, por ejemplo, sobre la medicación que toma una persona candidata o sobre ingresos hospitalarios previos vinculados a su discapacidad. Se trata de cuestiones que no guardan relación alguna con la capacidad para desempeñar el puesto de trabajo y que, además de generar una fuerte sensación de distancia y desconfianza, resultan contrarias a la legislación laboral y de protección de datos. En este sentido, aunque un 41,4% afirma que nunca le han planteado este tipo de cuestiones, un 41,9% indica que las ha recibido en alguna ocasión y un 16,5% con frecuencia. En otras palabras, un 58,4% de las personas encuestadas reconoce haber experimentado, en algún momento de su trayectoria, preguntas estigmatizantes en entrevistas de trabajo, vinculadas a su diagnóstico de salud mental.
Por otra parte, la mayoría de las personas con discapacidad psicosocial (55,6%) afirma que no necesita ajustes específicos en las entrevistas de trabajo. Sin embargo, un 44,4% sí los requiere, ya sea en forma de más tiempo, acompañamiento o formatos accesibles. En estos casos, apenas un 7% consigue que su petición sea aceptada, mientras que un 8,2% se encuentra con un rechazo. Asimismo, un 29,2%, pese a necesitar estos ajustes, no se atreve a solicitarlos por miedo a reforzar el estigma.
“La Ley General de Discapacidad establece con claridad la obligación de ofrecer ajustes razonables -ya sea en las pruebas, en los tiempos o en el formato de la entrevista- a las personas con discapacidad durante los procesos de selección. No hacerlo supone una forma de discriminación, aunque a menudo se presente de manera velada más que como un rechazo explícito. En la práctica, la falta de adaptaciones puede colocar al candidato con discapacidad por motivos de salud mental en desventaja, experimentando desorientación si las pruebas no se adaptan a sus necesidades, lo que limita sus posibilidades de demostrar sus competencias reales”, destaca Francisco Mesonero, director general de la Fundación Adecco.
La falta de acompañamiento impulsa el miedo a perder el empleo
Una vez las personas con discapacidad psicosocial acceden a un puesto de trabajo, el seguimiento tras la
incorporación es determinante para su estabilidad, pero un 58,6% manifiesta insatisfacción: un 17,3% recibe seguimiento de forma esporádica o poco útil y un 41,3% no cuenta con ningún acompañamiento. En contraste, solo un 28,2% valora positivamente haber tenido un seguimiento frecuente y eficaz.
Ajustes más demandados en el puesto de trabajo
¿Qué tipo de ajustes requieren con mayor frecuencia las personas con discapacidad psicosocial en su puesto de trabajo? La mayoría de las personas encuestadas (38,5%) demanda ajustes relacionados con los tiempos de trabajo, como disponer de un ritmo de aprendizaje progresivo o cierta flexibilidad en momentos de crisis. Un 32,5% alude a ajustes vinculados a sus funciones, como adaptar tareas según puntos fuertes o establecer objetivos claros y realistas. Además, un 14% destaca la necesidad de mayor sensibilización del equipo y de los responsables, de manera que exista un entorno de trabajo más comprensivo y libre de prejuicios.
En la práctica, sin embargo, solo un 30% afirma haberse beneficiado de estos ajustes, mientras que un 33,4% declara lo contrario: un 13,4% cree que fueron insuficientes y un 20% indica que no los recibió a pesar de necesitarlos. Estos datos muestran que el seguimiento y los ajustes razonables son todavía una asignatura pendiente en muchas organizaciones, pero también señalan un camino claro de mejora para avanzar hacia la sostenibilidad laboral de las personas con discapacidad como consecuencia de un diagnóstico de salud mental.
“Garantizar la estabilidad y la continuidad en el empleo para las personas con discapacidad de tipo mental requiere abordar tanto los factores organizativos (claridad, carga, apoyos, ajustes) como los factores actitudinales (prejuicios y posibles conflictos). De lo contrario, seguiremos viendo cómo, pese a los esfuerzos para contratar a más personas con discapacidad psicosocial, muchas de ellas no logran consolidar sus trayectorias laborales debido a un seguimiento inadecuado. Cuando se implementan ajustes, notamos un cambio notable, al permitir que el profesional con discapacidad desarrolle su trabajo según sus capacidades, y evitando situaciones desbordantes que podrían desencadenar una crisis. Por el contrario, la denegación de ajustes puede traducirse en un deterioro del rendimiento y, finalmente, en la pérdida del empleo”, explica Jordi Navarro, consultor de Inclusión especialista en Salud Mental de la Fundación Adecco.
Inseguridad laboral
La falta de acompañamiento y ajustes repercute directamente en la seguridad psicológica en el puesto de trabajo. Un 55% de las personas encuestadas reconoce haber experimentado dificultades para mantener su puesto debido a la falta de claridad en las funciones, al escaso acompañamiento de supervisores o compañeros, o a la indefinición de su rol en la empresa. A esto se suman otras dificultades frecuentes: sobrecarga de trabajo (27,8%), circunstancias externas como problemas de salud o familiares (26,1%) o conflictos o mal ambiente laboral (21,7%).
Falta de claridad en las funciones, acompañamiento difuso o rol mal definido (lo señalan un 55% de los encuestados). Cuando una persona no tiene bien delimitadas sus tareas o no sabe a quién recurrir ante dudas, es fácil que surjan malentendidos y estrés. La OMS destaca que las funciones laborales poco
definidas son un factor de riesgo psicosocial que puede afectar negativamente la salud mental en el trabajo.
Carga de trabajo excesiva u organización deficiente (27,8%). Algunas personas con discapacidad
psicosocial terminan desbordadas no tanto por su condición médica, sino por sobrecargas estructurales del puesto de trabajo. Jornadas excesivamente largas, ritmos acelerados o falta de personal de apoyo conducen a un estrés crónico. Este aspecto también está identificado por la OMS: las cargas de trabajo demasiado elevadas y la escasez de control sobre las tareas constituyen riesgos para la salud mental de cualquier profesional, intensificándose en el caso de aquellos con discapacidad por motivos de salud mental.
Dificultades externas al propio trabajo (26,1%). Aquí entrarían problemas de transporte, incompatibilidad de horarios con necesidades de tratamiento médico o apoyo familiar, trámites burocráticos, etc. Por ejemplo, si una persona necesita acudir regularmente a terapia y su horario laboral es inflexible, puede generarse un conflicto que ponga en peligro su continuidad.
Conflictos laborales (21,7%). Desavenencias con jefes o compañeros, situaciones de acoso laboral
(mobbing) o entornos poco acogedores pueden precipitar la salida. Las personas con trastorno mental a
veces se enfrentan a estereotipos en el lugar de trabajo que alimentan esos conflictos (por ejemplo, ser
consideradas “problemáticas” de antemano).
Como consecuencia. un 66% de las personas con discapacidad psicosocial que trabajan manifiesta temor a perder su empleo, un 45,1% de forma intensa y un 20,9% de manera parcial.
En palabras de Begoña Bravo, directora de Inclusión de la Fundación Adecco “El sentimiento de inseguridad podría mitigarse -e incluso desaparecer- si se trabaja bien esta fase crucial de seguimiento. Hay personas con discapacidad de tipo mental que no superan el periodo de prueba por cuestiones que podrían resolverse con ajustes sencillos o con una comunicación más fluida. Una mala experiencia no solo pone en riesgo su empleo, sino que también puede reforzar el estigma, y es precisamente lo que debemos evitar. Desde la Unidad de Apoyo a la Inclusión de la Fundación Adecco trabajamos precisamente por garantizar la correcta adaptación de la persona con discapacidad su puesto de trabajo, detectando las necesidades tanto del profesional como de la empresa”.
Por su parte, Francisco Mesonero, director general de la Fundación Adecco, subraya: “Es fundamental trabajar el liderazgo inclusivo, porque es la base que convierte una contratación en una oportunidad real de futuro. Los responsables de equipo deben ser quienes den ejemplo, con gestos y prácticas que transmitan accesibilidad, seguridad, claridad y respeto hacia todas las personas. Cuando un responsable reconoce que cada persona tiene sus propios ritmos, explica las expectativas con cercanía y habilita espacios de diálogo, ayuda a que la persona con discapacidad psicosocial sienta que pertenece y que puede dar lo mejor de sí misma. Todo ello reduce el estrés de adaptación, facilita que se pidan los apoyos necesarios y evita salidas que podrían haberse prevenido.”
Conclusión: empleo, inclusión y salud mental
El informe pone de manifiesto que el empleo, más allá de ser una fuente de ingresos, constituye una poderosa herramienta para la salud mental. Cuando se desarrolla en un entorno inclusivo, con apoyo y condiciones adecuadas, aporta estructura a la vida diaria, refuerza la autoestima, genera sentido de utilidad y pertenencia, y abre la puerta a relaciones sociales enriquecedoras. De esta forma, el trabajo se convierte en un factor protector y terapéutico, capaz de prevenir el agravamiento de los diagnósticos de salud mental y favoreciendo la plena inclusión social. No obstante, cuando el proceso de acceso o el propio desempeño está marcado por barreras, prejuicios o falta de acompañamiento, el empleo puede perder este valor e incluso transformarse en un riesgo para el bienestar psicológico.
“Cuando el proceso de búsqueda de empleo se desarrolla en un entorno adecuado, con una acogida respetuosa, acompañamiento y un trato inclusivo, las personas con discapacidad por motivos de salud mental tienen la oportunidad de desplegar su potencial y fortalecer su autoestima.
